How a breaking story develops

A breaking file starts the moment a primary action is confirmed — a vote tallied, a filing docketed, a closure announced. The first file carries what we can verify and a timestamp. As the document trail fills in, the piece is updated in place and the modified date moves. Nothing runs as fact before it traces to a source we can name; see the methodology for the full process.
